Steps for Requesting Housing Accommodations at the Hatfield Marine Science Center

1. Apply with Disability Access Services (DAS). 

2. Based on the information provided in your application, DAS may request the DAS Housing Documentation Form http://ds.oregonstate.edu/forms filled out by a qualified healthcare professional. This document can be faxed to 541-737-7354, emailed to [email protected]], or sent via regular mail to the DAS office. 
3. Based on the information provided in your application, DAS may request the DAS Assistance Animal Form http://ds.oregonstate.edu/forms filled out by a qualified healthcare professional. This document can be faxed to 541-737-7354, emailed to [email protected], or sent via regular mail to the DAS office. 
4. Once DAS receives the information above, we may request a 60-minute Zoom appointment with a DAS Housing adviser to discuss your disability-related impacts in a residential environment and your housing accommodation request. 
5. If you are requesting a Service or Assistance Animal at OSU, please review OSU Service & Assistance Animal Policy at http://oregonstate.edu/accessibility/serviceanimalpolicy.

DAS PROCESS:  

• DAS will send a reminder email to the student to complete the steps listed above once a housing accommodation request has been submitted
• Once provided with appropriate information for an OSU on-campus housing accommodation request, the DAS Housing Review Team will process and review the request. DAS will inform the student of the outcome of their housing accommodation request(s) through their OSU e-mail.
• Housing accommodations for Hatfield Marine Science Center are granted by DAS for a specific academic term(s).
