Steps for Requesting Housing Accommodations on the Cascades Campus

  1. Apply for housing through the OSU Cascades Residential Education and Housing page. 
  2. Complete the OSU-Cascades Housing Disability Accommodation Form. 
  3. Apply with Disability Access Services (DAS). 
  1. Based on the information provided in your application, DAS may request the DAS Housing Documentation Form http://ds.oregonstate.edu/forms filled out by a qualified healthcare professional. This document can be faxed to 541-737-7354, emailed to  [email protected], or sent via regular mail to the DAS office. 
  1. Based on the information provided in your application, DAS may request the DAS Assistance Animal Form http://ds.oregonstate.edu/forms filled out by a qualified healthcare professional. This document can be faxed to 541-737-7354, emailed to  [email protected], or sent via regular mail to the DAS office. 
  1. Once DAS receives the information above, we may request a 60-minute Zoom appointment with a DAS Housing adviser to discuss your disability-related impacts in a residential environment and your housing accommodation request. 
  1. If you are requesting a Service or Assistance Animal at OSU, please review OSU Service & Assistance Animal Policy at http://oregonstate.edu/accessibility/serviceanimalpolicy. 
  2. Please review the additional DAS Animal Guidelines for information regarding animals on the OSU campus(es). 

DAS PROCESS: 

  • Once the student has submitted a request for a housing accommodation (Step 2 above) the student will receive an email from DAS reminding the student to complete all the steps listed above (Steps 3 through 6).
  • Once provided with appropriate information for an OSU On-campus housing request the DAS Housing Review Team will process and review the request. DAS will inform students of the outcome of their housing accommodation request(s) through e-mail.
  • Housing accommodations are granted by DAS for an academic year (September – August of each year or the remainder of the current academic year).

SUMMER HOUSING ACCOMMODATION REQUESTS: 

If a student has not previously lived in on-campus housing with accommodations or is requesting a change to previous eligible housing accommodations then the above identified steps for requesting a housing accommodation must be followed. 

IF YOU ARE A RETURNING OSU STUDENT LIVING ON-CAMPUS ACADEMIC YEAR: 

All students with currently approved housing accommodations through DAS will receive a reminder from UHDS regarding the housing application deadline, reminding them to request their housing accommodations for the upcoming academic year.

If a student is requesting the same housing accommodations that were approved for the current/previous academic year, the student needs only to submit a Disability Accommodation Form (Step 2).

Continuously enrolled students who miss the established UHDS deadline for requesting on-campus housing will lose their priority selection status. This includes returning students who are registered for services with DAS who are requesting housing accommodations for the upcoming academic year. It’s important to get the housing application and housing accommodation requests in as soon as you can.
